Clinical Aspects of the Preoperative Selective Angiography in Patients with Pancreas and Periampullary Tumors



Abstract
Fourty-eight patients with pancreatic and periampullary tumors staged by CT scan or visceral angiography who received operation in the Department of Surgery at kuro hospital, Medical colleage of Korea University, from September 1983 to December
1989,
were reviewed to increased accuracy of the resectability with the aid of angiogram.
@ES The results obtained were as follows :
@EN 1) Male to female ratio was 1.8 : 1, the most prevalent age was between 61 to 70 (42%), and mean age was 57.5 years old.
2) The most commonly occuring lesion was pancreas head in 25 cases (52%) followed by distal CBD in 10 cases (20.8%), and ampulla of Vater in 7 cases, and duodenum in 4 cases.
3) Among the 30 patients without CT scan evidence for metastatic disease, 22 patients underwent a resection, for a resectability rate of 73%.
4) Among the 20 patients with normal angiograms, 15 patients underwent a resection, for a resectability rate of 75%.
5) Twenty patients staged by CT scan and believed to be resectable, were staged further by visceral angiography, Visceral angiography was normal in 17 patients and 15 patients underwent a resection, for a resectability rate of 88%.
6) All of the 15 patients, Whipple's operation were performed in 12 cases(80%), total pancreatectomy in 2 cases, regional pancreatectomy in 1 case.
